If medical certificate is a must, then let the authorities put up the information on what are those medical conditions which can disqualify a person from getting a license.
Data sharing and thereafter mining may be done, from the exercising collating the data, for better service efficiencies utilising artificial intelligence. Of course, the customer may optionally choose to share his or her data. No insurance claim honoured, unless one has a valid driving license, being such an application of information sharing.
The Doctor Certificate is sometimes mandatory for vision, colour blindness , Fits etc. But give this jobs to Govt. Hospitals for Certification. Each test takes 15-20 minutes. Give appointment to 25-30 peoples per day per ward hospital. It will solve the problem.
Not ideal, but optimal, the solution here! Even the doctor who charges Rs.300 would just put his/her stamp, thereby profiteering from a formality! Nonetheless, this remains an attention directing and record keeping exercise, at least. Eventually, see how much better than before, becoming technologically assisted, have the licensing authorities become. Minimum human discretion, meaning bribery, involved now.
