How to claim PF that is pending for many years

My wife was working in Zion hospital Mumbai from 1995 to 2000 as a nurse. Out of somany years only for 3 years she worked as a permanent staff. After that she resigned and moved to Cochin for marriage purpose. Even after resignation almost for 7-8 months salary kept coming into her account which she was not aware of. But then later after getting to know about this she returned the excess money back to her office. But she couldn't proceed with the PF formalities as she had to move to Cochin. After somany years how do we carry on the procedures. Please advise the simplest way of getting it back since we are in a far off city from Mumbai. more  

For withdrawal you need to submit both Form 10C and Form 19. You can download both the forms from EPF website more  
I worked in Gayatri Vidya Parishad Degree College for nearly 5 months and under gone accident in 2012. In 2015 I worked in Peppertap e-online grocery store for 2 months.It was closed due to lack of bussiness in Visakhapatnam.TMay I know the way how to get epf from both the organisations. more  
If your wife is in employment at Cochin, she can very well get the PF contributions of previous organisation transferred to her present employer. It can be done by submitting Form 13 (R) duly signed by the authorised person of the present employer. If she is not in service send Form 10(C) for withdrawal through her previous employer. more

Kindly send the following documents with the attached PF Claim Forms to the employer. 1. Cancel Cheque copy (Should be saving account & having the IFS Code) 2. Bank Pass Book Xerox copy (if name not printed on Cheque) 3. Pan card copy (Compulsory) 4. Address proof. 5. Form-15G/Form-15H (Whichever Applicable) (2 copies) 6. Mention UAN in the forms.
